MIR 2028 in Spain: a guide for US medical graduates
In Spain, residency starts with a single national exam, the MIR. If you earned your MD in the United States and want to train in Spain, here is the path, step by step, with the official sources.
The MIR is taken in Spanish, and so is the VIRTUAMIR prep: you need a strong command of the language (C1 level).
Timeline
MIR 2028 key dates
- Registration
- September 2027 (estimated)
- Exam
- Late January 2028 (estimated)
The MIR 2028 dates are estimated from previous years: registration opens in September and the exam is held on a Saturday in late January. Official dates are published in the BOE with the call (the MIR 2027 call came out on August 10, 2026).
Already admitted to the MIR 2027? It’s on January 23, 2027: 95 days to go.
Step by step
Step by step (United States)
Get your MD homologated in Spain
Your degree must be homologated in Spain before MIR 2028 registration closes. You apply online through the Spanish Ministry of Science, Innovation and Universities, under Royal Decree 889/2022.
Gather your diploma, your transcript with hours for each course and clinical rotation, and your school’s curriculum.
Apostille and sworn translation
The United States is party to the Hague Convention, so your documents get an apostille from the relevant state authority, with no consular legalization. Since they are in English, you also need a sworn translation into Spanish.
Spanish at C1 level
The exam is entirely in Spanish. Because Spanish is not an official language in the US, the call requires an official Spanish certificate at C1 level or higher, unless you studied medicine in Spanish.
Places for candidates without Spanish residency
Without a residence permit in Spain, you compete for the places reserved for that group: 968 of the 9.676 medical places in the MIR 2027. The call also requires your country to have a cooperation agreement with Spain in force; check it in your year’s call.
With legal residency in Spain, or Spanish or EU citizenship, you compete for every place.
Register in September, sit the exam in January
Registration takes place in September with form 790 (a 31,10 € fee in the last call). The exam is in person, in Spain, on a Saturday in late January.
Comparison
The MIR vs. the USMLE and the Match
In the US, you match into residency through the USMLE, applications, interviews and the NRMP Match. In Spain there are no interviews or letters: one national exam ranks every candidate, and each one picks a place in rank order.
What changes:
- One score: the exam (90 %) plus your academic record (10 %).
- Pick by rank from every place in Spain, with a public list.
- The format: 200 preguntas y 10 de reserva, four options each, in cuatro horas y media, in Spanish.
- A contract and a salary throughout residency, in the public system.

FAQ
Frequently asked questions
Do my USMLE scores count?
No. The MIR only counts its own exam and your academic record. What you studied still helps: much of the clinical content overlaps.
I have a DO degree: can I apply?
Homologation is assessed case by case against the Spanish medical degree. Check with the Ministry of Science, Innovation and Universities before you apply.
Which Spanish certificate is accepted?
An official certificate at C1 level or higher, such as the DELE or SIELE from the Instituto Cervantes. Check the call for the accepted list.
Are there places reserved for Americans?
Not by country: in the MIR 2027, the 968 places for candidates without Spanish residency were awarded by score across all nationalities.
Can I become a Spanish citizen?
The general requirement is ten years of legal residency (Spanish Civil Code, article 22). Residency alone already lets you compete for every MIR place.
How should I prepare for the MIR?
With official questions from recent years, in Spanish, and the notes that explain them. VIRTUAMIR links them together, and you can start for free.
